Appointments
If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. Appointments missed or canceled with less than 24 hours’ notice may incur a fee, which is not covered by insurance.
Fees & Payments
Payment is due at the time of service, excluding charges your insurance company is contractually responsible for. For your convenience, we accept cash, American Express, MasterCard, Visa, Discover, and money orders. Personal checks are generally not accepted. We understand emergencies do arise. If you anticipate a delay in your payment, please contact our Billing Department for assistance.
Form Fees
If you require a form to be completed by one of our physicians (e.g., FMLA, Disability, AFLAC), there is a fee of $50 per form. This fee must be paid prior to completion. Please allow adequate time, as physicians’ office schedules vary.
Medical Record Requests
Paper reproduction fees are $1.00 per page for the first 25 pages, and $0.25 per page for pages 26 and up. Electronic records sent digitally are charged a flat, reasonable, cost-based fee in compliance with HIPAA. Images provided on a compact disc (CD) are $20.00, plus an additional $3.00 if mailed.
Prescriptions Refill Requests
Prescription refill requests must be made Monday through Friday, 9:00 a.m. to 4:00 p.m. Please note that requests made after 4:00 p.m. may not be reviewed until the following business day. Please allow 48 to 72 hours to process your refill request, as physicians’ office schedules vary.
Returned Check Fee
Returned checks will result in service charges up to the maximum amounts permitted by Florida law.
What to Bring to Your Appointment
If you are a new patient, please arrive to your appointment early in order to complete the registration process. In addition, please bring the following:
- Driver’s License or State ID
- Referral (If Required by Insurer)
- Insurance Information
- Copies of Operation Records, Medical Records, X-rays, MRIs, and CT Scans from Prior Doctor Visits
- A List of Current Medications and Any Known Allergies
